ACR Form for the year 2008-09 are available with CA to CMS. Please get it collected, fill out the self appraisal part and return it within 3 working days.
The Reporting Officer for the year 2008-09 shall be Dr. S. K. Gadi and Dr. B. B. Aggarwal.
A suggested list of items to be covered in the ACR Form has been prepared by experts which is available at following link:

Although comprehensive the list is by no means exhaustive, any suggestion for improvement is welcome.
