Please bring the completed Data Sheet, on the day of Monthly Meeting and submit it to CA to CMS/Dr. S. P. Chadhary. In case of Multi Doctor Health Unit the Doctor attending the Meeting should bring all other's sheet. The compiled Data is to be submitted to HQ.
