A Meeting will be held in Divisional Hospital of all the H & MI of colonies on 07.10.2008 at 1100 Hrs. A Fact Sheet has been developed. It is to be filled for each of the colonies separately. The Fact Sheet and the 'Instructions to fill it' can be downloaded from the link below:

The Filled up Fact Sheet for each Colony has to be submitted by the respective H & MI on 07.10.2008.
