Mid Term Appraisal
Mid Term Appraisal forms are available in the CMS Office. Scanned copies of the form will be mailed to those, who have given a test mail on arunmha@yahoo.co.in. They can download it and take out the print. Otherwise you will have to get it collected from the CMS Office. CMS desires that the completed form should be deposited in his office within three days.
