CHECK LIST FOR REIMBURSEMENT CLAIM ON MEDICAL EXPENCES

KINDLY ATTENTION all RAILWAY EMPLOYEES ( SERVING & RETIRED) CLAIMING RE-IMBURSEMENT OF MEDICAL EXPENCES/ all AMO’s, & HU In charges forwarding files.

For timely & speedy disposal of cases claiming reimbursement of expenses on medical treatment in ‘EMERGENCY CIRCUMSTANCES’, at the level of Divisional Hospital, Delhi, kindly scrutinize your all papers/documents at your level before submitting at Divisional Hospital, Delhi , and attach following documents with the Performa claiming reimbursement, so that file can be forwarded to higher authorities on time without delays occurring due to non availability of documents or due to inability to verification of reports/bills/vouchers.

1. Kindly see your treatment papers, that you are submitting your application for claiming reimbursement on medical expanses within the prescribed time limit , i.e. within six months of the treatment taken. If you submitting your papers after six months, kindly give an application stating reasons for the delay, and first get your case ‘Delay Condon’ from Accounts Department, and then submit it at Divisional Hospital, Delhi ( This will save your a lot of time).

2.Railway Employees/ Retired Railway Employee’s application , addressed to CMS, Divisional Hospital/DLI, giving circumstances under which he/she took treatment. (Kindly do not forget to give your clear address and valid phone number in working , where you can be contacted in future whenever required.)

3.Photocopy of medical card/ RELHS I Card( all pages,Dully attested by the Gazetted Officer).

4. Essential Certificate issued by the treating doctor of the hospital, ( not by casualty doctor ) countersigned by Medical Supdt. Of the the Treating Hospital .

5. Discharge certificate slip in original. Kindly also submit all reports in original or photocopy of investigations/ procedures done during the treatment. This will help in verifying the bills/ vouchers attached with the application.

6.Bills/ Vouchers submitted in Original, dully verified & countersigned by treating doctor, ( Authorized Medical Officer)( not by casualty doctor).

7.Detailed item wise break up of all the bills( this means , all bills /vouchers submitted above should be reproduced in legible manner ), e.g

10. All AMO’s/ HU In charges to kindly give attention on above points before forwarding the application of patients claiming reimbursement.

11. Dealing OS at Divisional Hospital to kindly receive and submit the files for further action only after checking all points as mentioned above for speedy & timely disposal of files of reimbursement.

REIMBURSEMENT OF MEDICAL EXPENSES- PROCEDURE OF DISPOSAL

Copy of RAILWAY BOARD’s letter no No. 2005/H/6-4/Policy-II dated: 31.01.2007

The issue of bringing in objectivity, consistency and transparency in disposal of reimbursement cases, where the treatment has been taken in emergency without consultation with the Authorized Medical Officer, has been under consideration of the Board

The Railway Board after thorough review of the whole subject of reimbursement has taken the following decisions which are to be implemented with immediate effect.
Any instructions on this subject as available in IRMM 2000 or any office order issued prior to this office order and will stand modified accordingly.

I The cases to be considered for sanction of reimbursement claim

To provide proper medical treatment, the Indian Railway Health Care Delivery system has 121 number of Railway Hospitals and 586 No. of Railway Health Units established all over India. In addition to this, all Govt. Hospitals and more than 115 private hospitals all over the country have been recognized to provide necessary medical treatment to Railway beneficiaries.
As per extant rules, a railway beneficiary must report to Railway Medical Officer for his/her and dependents’ medical treatment. The Authorized Medical Officer will make necessary arrangements for medical treatment through Railway Hospital/Govt. Hospital / Pvt. Recognized Hospital. In exceptional situations, CMOs of Zonal Railways can obtain special permission from Railway Board for treatment in any Private Hospital on case to case basis. Hence, there is no scope available for any railway beneficiary to go to any private hospital himself/herself or their dependents on their own volition, except in case of real emergency situation.

“Emergency” shall mean any condition or symptom resulting from any cause, arising suddenly and if not treated at the early convenience, be detrimental to the health of the patient or will jeopardize the life of the patient. Some examples are- Road accidents, other types of accidents, acute heart attack etc. Under such conditions, when the Railway beneficiary feels that there is no scope of reporting to his/her authorized Railway Medical Officer and avails treatment in the nearest and suitable private Hospital, the reimbursement claims are to be processed for sanction, after the condition of the emergency is confirmed by the authorized Railway Medical officer ex-postfacto.

In order to establish the emergency condition, following parameters are to be examined on record:-

(a) Admission details:-

(i) Date and time of admission.
(ii) Admitted through OPD service/ emergency service.
(iii) Admitted to an ICU bed or general bed or cabin bed.

(b) Clinical findings at the time of admission. Following findings should be made available and critically evaluated:-

(i) Pulse rate.
(ii) B.P.
(iii) Level of consciousness
(iv) Any convulsive feature.
(v) Urine output (vi) Any other feature of shock.
(vii) Body temperature
(viii) Extant of external wound
(ix) Extant of active bleeding.
(x) Extant of Chest pain or pain in other parts of the body.

I. Types of medical treatment given immediately after admission.

(i) List of Emergency medicines used immediately after admission.
(ii) Type of surgical procedure done immediately after admission.

II. Calculation of the amount of reimbursement to be sanctioned out of the claimed amount:-

Once the emergency is established beyond doubt, then the case should be further processed for calculating the amount/money to be sanctioned.
For that, following guidelines are given:-

a) Treatment taken in Govt. Hospital – Full admissible amount should be recommended for sanction.
b) Treatment taken in Recognized Private Hospital for an ailment for which it is recognized – Rate as approved by Railway should be processed for sanction.
c) Treatment taken in a Recognized Private Hospital but for an ailment for which it is not recognized or Treatment taken in a non- recognized Private Hospital:-Reimbursement should be made at the CGHS rates of that city or nearest city. CGHS (Central Govt. Health Scheme) approved rates are to be recommended/processed as an upper limit for sanction.

III. The approved rates of private hospitals recognized by Railway should be put on the Web sites of the Zonal Railways so that at all levels of processing/sanction these are accessible to all concerned.

IV. In Medical Science, no list can be fully exhaustive. Hence, it is likely that there will be few occasions when a claim has been submitted which is not appearing exactly in the CGHS rate list. On these cases, the MD/CMS/MS in charge of Divisions will apply their mind and will come to a logical conclusion. Then, they will pass a speaking order to certify the rate/s being recommended, in consultation with Associate Finance.

V. All the reimbursement cases be processed as per check-list issued earlier to avoid back reference.

VI. These instructions shall be effective from the date of issue of this letter. Past cases, already decided, shall not be re-opened.

Q (1). Whether following conditions are considered to be an “emergency” by the Railways, for reimbursement of medical expenses for treatment in Private/Government Hospitals;

CONDITIONS OF EMERGENCY – UNDER ECHS

* Acute Cardiac Conditions/Syndromes including Myocardial Infaraction, Unstable Angina, Ventricular Arrhythmias, Paroxysmal Supra-ventricular Tachycardia, Cardiac Tamponade, Acute Left Ventricular Failure/Severe congestive Cardiac Failure, Accelerated hypertension, Complete dissection.
* Vascular Catastrophes including Acute limb ischemia, Rupture of aneurysms, medical and surgical shock and peripheral circulatory failure.
* Cerebro-Vascular Accidents including Strokes, Neurological Emergencies including coma, cerebro meningeal infections, convulsions, Acute paralysis, Acute visual loss.
* Acute respiratory Emergencies indulging respiratory failure and decompensated lung disease.
* Acute abdomen including acute obstetrical and gynecologist emergencies
* Life threatening injuries including Road traffic accidents, Head Injuries, Multiple Injuries, Crush Injuries and Thermal Injuries.
* Acute poisoning and Snake bite
* Acute endocrine emergencies including Diabetic Ketoacidosis
* Heat stroke and cold injuries of Life threatening nature
* Acute Renal Failure
* Severe infections, leading to life threatening sequel-including Septicemia, disseminated TB.
* Any other condition, in which delay could result in loss of life or limb.

Ans. All the conditions enlisted in the application as “emergency” (under ECHS) are considered as emergency conditions by Railways also.

While issuing the circular No. 2005/H/6 4/Policy-I1 dated 31.01.07 by Railway administration, all the conditions (under ECHS) have been included and considered by defining in a separate pattern. The pattern in which they are considered is given in the above mentioned circular.

The copy of the said Board’s letter dated: 31.01.07 is attached (Annexure ‘A’) for ready reference.

GM XI Vs. DRM XI

A Cricket Match was played at Karnail Singh Stadium on 20.02.2010 between GM XI and DRM XI. Mr. Ashwani Lohani, the captain of DRM XI won the toss and elected to bat first. Mr. Vivek Sahai, the fielding captain, led from the front and bowled two restrictive overs demontrating great control over the line and length. He mobilised his men well to restrict the the batting side to a modest score of 125 in the allotted 25 overs, a score which was overhauled by GM XI rather easily. The atmosphere in the stadium was that of a carnival, a lot of bonhomie was there on display. In Line of Fire of a Different Kind

not be occupying the centre place in our consciousness as being engaged in a dangerous work, but one has to see a S. Karamchari descending into a deep manhole to appreciate the kind of risk these humble men take to keep our homes comfortable and hygienic to stay. A manhole may look deceptively safe but when thousands of litres of water and muck gush in from a blocked line after the blockage is removed even the reflexes of a top sportsman may fail to take him to the safety of the ladder. So every time a Railway man descends into a manhole he is made to wear a safety belt through which his colleagues can pull him to safety in case of some unexpected happening.

Courage Personified

This is the photograph of a 4 year old boy who apparently lost his parents and elder sister in the train accident on 20.10.2009 near Mathura Junction. Goa Express had rammed into Mewar Express in the wee hours of morning. This boy was found with right lower limb traumatically amputated. As no adult came forward to tell about him it was presumed that all adults travelling with him were dead.

Yours truly was told this heart rending story on reaching the Methodist Hospital by the staff present there. Amazingly the boy was aware of his surroundings and oriented. He displayed unbelivable composure in his interaction with everybody. In fact it was he who told his name and the address of his Home. When his father turned up the relief on everybody's face was apparent. The author also said a silent prayer of thanks for this gloom lifting event.

Within a few minutes the boy became friendly and even waved a good bye when author was leaving the hospital.

This boy is Master Hitesh, a resident of Jaipur.

Notification of MCI on Medical Ethics

CMS/DLI desires every Medical Officer to go through the new Notification of MCI regarding Medical Ethics and ettiquettes available at above link. Some of the sailent points are reproduced below;

"................

a) Gifts: A medical practitioner shall not receive any gift from any pharmaceutical or allied health care industry and their sales people or representatives.
b) Travel facilities: A medical practitioner shall not accept any travel facility inside the country or outside, including rail, air, ship , cruise tickets, paid vacations etc. from any pharmaceutical or allied healthcare industry or their representatives for self and family members for vacation or for attending conferences, seminars, workshops, CME programme etc as a delegate.
c) Hospitality: A medical practitioner shall not accept individually any hospitality like hotel accommodation for self and family members under any pretext.
d) Cash or monetary grants: A medical practitioner shall not receive any cash or monetary grants from any pharmaceutical and allied healthcare industry for individual purpose in individual capacity under any pretext. Funding for medical research, study etc. can only be received through approved institutions by modalities laid down by law / rules / guidelines adopted by such approved institutions, in a transparent manner. It shall always be fully disclosed. "

Thyroid Surgery

"39 yr female patient presented in Delhi main hospital with c/o of goiter with hypothyroidism, MRI suggestive of multinodular goiter involving both lobe,
FNAC - only presence of follicular cells.
She was operated in Delhi Div. Hospital by Dr B.N Tiwary, Surgeon in Nov. 09
Surgery done was- Total thyroidectomy with preservation of parathyroid gland.
Post OP period was unevenful , no hypocalcemia.
HPE report was- Diffuse Lymhocytic Thyroiditis, intact total thyroid without parathyroid gland."

New Year Party

The staff and the Officers of Divisional Hospital, Delhi organised a New Year Party on 01.01.2010 in the Hospital premises. A lot of interaction took place and bonhomie was there on display in the party.

CMS, Dr. S. K. Gadi exhorted the personnel of Medical Department Delhi Division to work hard and sincerely and thus bring about a perceptible improvement in the image of the Department.

The New DRM

Mr. Ashwani Lohani has taken over as the DRM of Delhi Division on 24.12.2009. He is a 1979 batch officer of Mechanical Engineers cadre.
He has four engineering degrees.Fellow of the Institution of Engineers, India and the Chartered Institute of Logistics and Transport. Awards received : PATWA International Young Achievers Award.Ministry of Railways National Award. Two National Awards of Ministry of Tourism, Govt of India. “Ashwani Garden” was created at Patratu, Bihar.The Week magazine featured him as “Man of the Week” in 21st July 2002 issue. Set a Guinness World Record in 1998 by running the Fairy Queen Express. Piloted the nomination of the Darjeeling Himalayan Railway and the Mahabodhi Temple at Bodhgaya as World Heritage sites. tourism. As CMD ITDC achieved turnaround of the corporation. Recently as the MD of MPSTDC he achieved a turnaround of the corporation and brought the state in the forefront of Indian tourism. Wrote two books, one titled "Smoking Beauties", a coffee table book on steam locomotives and the other titled "Winning at Work against all odds", a book on management. Also authored a comprehensive report on "Rail Heritage and Tourism". Written over 60 articles on a variety of subjects in magazines of repute.

He is setting a scorching pace by undertaking inspections of Railway Stations and colonies extensively. The picture was taken by yours truly in RRI of Delhi station on 27.12.2009 during an inspection.

Meeting and the party on 11.11.2009

As informed earlier about the turbulent weather conditions, when the meeting started in CMS chamber at 1400 hrs the mood was sombre but as the meeting progressed and the new CMS chalked out his vision to the Medical Officers with a few inevitable lighter moments thrown in (circumstantially) the pal slowly lifted.

Everybody came into his/ her own really after reaching the venue of the party i.e. the drawing room of new CMS Dr. S. K. Gadi.

P.S. The 'Lighter Moment of the Day' Dr. Arora informed the house that his arrears have been made but sent in the name of Sr. DMO, Shamli. Instead of showing any sympathy to the plight of Dr. Arora, the whole house congratulated Sr. DMO, Shamli in unison.

All the Medical Officers are requested to make full use of the Monthly Meeting. Precious resources are utilised to organise these meetings. It is our duty to justify the expenditure by making it fruitful. CMS desires to see the following inputs (as applicable) from all the Medical Officers:

1. List of Persons on sick list for more than 15 days.
2. List of Persons suffering from Chronic Sickness like HT, DM, CAD with the medications they are receiving.
3. Plan to monitor the working of the health inspectors.
4. Duty list of Nurses & Staff working under the M. O.
5. List of recognised and 'potentially recognisable' hospitals for referral patients in emergency.
6. ARME Inspection Schedule.
7. List of Pending Reimbursement cases.
8. List of Pending D & AR and Vigilance cases.

CMS plans to conduct the meeting going through the lists one by one, please come prepared.

Monthly Meeting

The Monthly Meeting for the month of November shall be held on 11.11.2009 in the renovated Chamber of CMS Delhi from 1400 hours onwards.

The Plans for the Bash have been postponed for now keeping in mind the 'turbulent weather conditions'.

Vigilance Awareness Week

Vigilance Awareness Week is being celebrated in all the Government Departments. Railway has organised various events to mark the occasion. A function was held at Rail Museum on 05.11.2009 which was attended by all the senior officials of Railway including GM, AGM, SDGM.

CMS Delhi desires that all the Medical Officers should keep themselves updated about the various rules and regulations. With the new work profile the Department has become prone on some new fronts about which we all need to get sensitised. Efforts are made in this direction during monthly meetings but we all owe it to ourselves and the department to be 'vigilant' about not to invite any vigilance case.

As I said 04.11.2009 was a day of major developments. During the meeting in CMS Chamber news came that the SAG Promotion orders have been issued from the NR Head Quarter Office. The scanned list is uploaded for everybody's benefit.

Congratulations to everybody who has been promoted. Good news is that Delhi Division Medical Department is planning a big bash to celebrate the occasion.

New CMS

04.11.2009 was a day which brought major changes in the dispensation at NR Divisional Hospital, Delhi. Barely had anybody settled in his/her seat when the NEWS came that two SAG Officers of NRDH namely Dr. B. K. Mittal and Dr. A. K. Sengar have been transferred.

Dr. Mittal has been posted to Sub Divisional Hospital, Amritsar and Dr. Sengar takes over as CMS, Jagadhari. The new CMS of Delhi Division is Dr. S. K. Gadi.

The new CMS called a meeting in his room in the afternoon. The Doctors present in hospital attended it. The new CMS asked everybody not to be preturbed by anything and give their best to the job. He emphasized that nobody should get stressed, everybody has to look out for each other like a family member.

Very soon a full fledged meeting of all the doctors of Division shall be called, details are almost finalised but will be published on blog after confirmation.

Medical Department of Delhi Division is doing a commendable job in Giving the decision in cases of all the candidates sent for Medical Examinations in A1 for ALP within prescribed time period, a no mean feat considering the number of candidates being sent is close to 100 per day. Final figures shall be provided later on. In the meantime all of you can enjoy the unusual methodology adopted by Dr. Chatterjee.

A Healing Touch

Delhi Division was there in full strength to receive the passengers of 2443, Bhubaneshwar Rajdhani Express, which was held up by some miscreants in West Bengal for more than 5 hours. The team was led by DRM Delhi Sh. B. D. Garg. AGM, N. Railway, other Officials from Head Quarter of Norhtern Railway and Railway Board were also present. Medical Department of Delhi Division was there to render any medical assistance, if required, at the Platform one. CMD Northern Railway Dr. R. N. Tripathi was there and answered the questions posed by Media Personnel. NRCH Team was led by its MD Dr. Pandey.

All the passengers were greeted with a rose and were provided with water and other refreshments. Delhi Division had arranged for the luggage porters to carry the luggage of the passengers. Regular announcements were made to make the passengers aware of the First Aid Post and the Medical assistance available at Platform no. 1. Seven passengers came to the post for taking treatment, all were minor in nature.

The efforts were praised by all the persons present there.

An Achievement worth mentioning

Today a post is being published which should have been published in June 2009. Actually I thought not to publish it but than it was something for which the Doctors have been hired in IRMS so I thought, "Better late than never."

If you look at the photograph alongside carefully you will notice a piece of glass placed on a gauge piece (The photograph was taken with the help of the camera in cell phone). This glass piece was taken out from the foot of a gangman at Railway Health Unit, Sonepat. It was lodged in the foot of Hapless man for months and it took a lot of courage on part of the Doctor there to explore the nodule in a Health Unit on June 18, 2009 without anybody else to fall back upon. Eventually the smalto was succesfuly dissected out and the patient was relieved of fever and pain after a long time.

Delhi Division has won the shield for Best Achievement in the field of Health and Family Welfare in the year 2008-09. A small tea party was held in the chamber of Dr. S. K. Gadi to celebrate. On the Occasion both the CMS emphasised that it is due to the combined effort of Everyone. Dr. Mittal emphasised that the tempo needs to be maintained. Dr. Gadi said that the achievement doesn't belong to any single individual but the collective effort has been the driving force.

1. Reduction in number of sick certificates issued, man-day’s loss, imprest expenditure in percentage against corresponding period of last year.
2. No. of multipurpose camps, Health check up camps, Immunization camp, School health camps, IUCD insertion camps, Special cleanliness drives. Field action group meeting. Health education meetings on public health and alcoholism/AIDS/Hepatitis-B/TB in society with out put there of held against last year.
3. Innovation works undertaken, e.g. tree plantation, gardening, improvement in methods of cleanliness maintenance, renovation work, achievements in infection control/BMW mgt works, quick disposal methods of patients.

Can Anybody maintain this Sewer Line?

A Sewer Line was getting repeatedly choked. Despite best efforts the line could not be cleared. After estimating the approximate location, the HI decided to open up the patient (not laprotomy, it is called 'टांकी लगना'). After much huffing and puffing this is what he stumbled upon. Some Innovative Engineer had put a 2" pipe within a 4" pipe. Apparently the line had collapsed in the past and to get the line flowing this pipe had been passed.

This happens only in India!!

It is getting a bit heavy. Whenever one open the blog, either there is instruction from the CMS, a circular from Railway Board or an Inspection Notice. After the 'Big Boss' installed his picture in the style of 'Somebody Watching you' on the 'CORNER' it was felt that we ordinary mortals also require a little place for ourselves and .... whew than this big exhalation occurred.

Dr. Bhatia is requested to keep this space inhabited with the gags which he is known to generate. We will also try to keep pace by sharing jokes, anecdotes, the tempoo baba thoughts etc.
